2. PROGRAM REGISTRATION & PAYMENT

Registration is not complete until required payment is received. Pricing, deposits, payment schedules, and program requirements may vary by program, event, membership, or training option. Any additional terms presented during checkout also apply to that purchase.

3. CANCELLATIONS, REFUNDS & CREDITS

Cancellation, refund, rescheduling, and credit policies may vary depending on the program or training option purchased. Customers should review the applicable policy before completing registration. You should make sure the actual policy is clearly presented at checkout rather than leaving this completely open-ended.

4. PACKAGES & MEMBERSHIPS

Training packages, memberships, and hybrid plans are subject to the specific benefits, booking limits, expiration dates, and other conditions disclosed when purchased. Memberships that automatically renew should clearly state the renewal schedule, amount/frequency of charges, and cancellation process at checkout.

5. ATHLETE PARTICIPATION

Participation in 612 Football training and athletic activities involves inherent risks. Athletes and their parent/legal guardian, when applicable, must complete any required participation agreements, waivers, acknowledgments, and consents before participating.

6. MINORS & PARENT/GUARDIAN CONSENT

Athletes under 18 must have a parent or legal guardian complete registration and any required agreements or consents on their behalf.

7. HEALTH, SAFETY & CONCUSSIONS

612 Football may remove an athlete from participation when there is a health or safety concern. Concussion-related situations will be handled in accordance with applicable Minnesota requirements, including removal from participation and return-to-play requirements.

8. PHOTO & VIDEO

Photos and videos may be taken during 612 Football training, programs, events, and activities. The use of an athlete’s image or likeness for marketing, social media, website content, or promotional purposes is subject to the photo and video consent provided by the athlete or parent/legal guardian during registration.
